Applcation Options

Select your option of preference:

• Print, then fill out - (write in blanks)
• Fill out, then print - (type in blanks)

To print an application, set resolution to 1024 x 768, right-click on background and select "print." To properly use the "fill out, then print" form, first fill it out by typing in all the applicable fields, then print it.

IMPORTANT: Only what can be seen in the type-in blanks will print. Use extra paper, if necessary, and compose using your word processing software.

NOTE: These forms have been tested. If used according to instructions, they work correctly. If you have difficulty using them, please contact the church for assistance or to request an application be mailed to you.
